Advance Health Care Directive (AHCD) document performs three important functions:<\/strong><\/p>\n

First, the Directive identifies health-care agents who are to make health-care decisions for you if you are unable to make those decisions.<\/p>\n

Secondly, the Directive specifies your wishes in the event you are in a terminal condition and are unable to voice your wishes regarding your continued health-care.<\/p>\n

Finally, the AHCD allows you to elect to be an organ donor.<\/p>\n
